Alpine Medical and Specialty Practices has joined with Pagosa Springs Community Center to expand health and wellness services in Archuleta County. The program offers area seniors wellness checks and blood pressure monitoring or allows individuals to discuss two health related subjects of their choice. The outreach clinic will be held at the Pagosa Springs Senior Center on various dates each month. Sign-in sheets will be available for pre-scheduling. Interested parties should schedule a time by contacting the Senior Center at 970-264-2167. There is no charge for Medicare enrollees. Participants should bring identification, including their Medicare card, photo ID, and if necessary any supplemental insurances. Medicaid is not accepted at this time. Participants are encouraged to bring a list of their current medications.
Clinical assessment will be provided by Tabitha Zappone, FNP-C of Alpine Medical and Specialty Practices (AMSP). Tabitha is a compassionate provider, invested in serving the community of Pagosa Springs. She understands the difficulty obtaining health care in a rural areas and how these obstacles can be greater for senior citizens. The goal of the outreach clinic is to provide care to those who are unable to travel or have other extenuating considerations.
